Highland Medical Center is seeking a compassionate and experienced Care Manager to help coordinate care for patients with chronic conditions and support positive health outcomes across our community.

In this important role, you’ll work closely with providers, patients, and community resources to develop individualized care plans, identify gaps in care, and ensure patients receive the support they need to manage their health. By building strong relationships with patients and collaborating across the care team, you’ll help advance HMC’s mission of providing quality care close to home.

What You’ll Do:

  • Assess patients’ chronic conditions and develop individualized care plans

  • Collaborate with providers to coordinate patient care and communicate treatment plans

  • Perform pre-visit planning and review patient history to identify gaps in care

  • Educate patients and families about treatment plans, chronic disease management, and preventive care

  • Monitor patient progress and update care plans as needed

  • Assist patients with transitions of care following hospitalizations or changes in treatment

  • Support Medicare Annual Wellness visits and other preventative care initiatives

  • Document patient interactions and care coordination activities in the medical record

  • Connect patients with community resources and outside healthcare partners when needed

  • Serve as a resource for patient education related to chronic disease management

  • Assist with patient care and emergencies as needed

  • Support team-based care through communication, collaboration, and care coordination

What We’re Looking For:

  • Graduate of an accredited nursing program

  • Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the Commonwealth of Virginia

  • Minimum of three years of experience in a primary care, psychiatric, or community-based healthcare setting preferred

  • Experience working with patients across the lifespan preferred

  • Strong communication, organization, and patient education skills

  • Proficiency with electronic health records and Microsoft Office

  • BLS, ACLS, and PALS certifications required
